Best Care Heart Health Program
The Best Care Heart Health Program provides comprehensive assessment, education, and ongoing support for individuals living with atrial fibrillation (AFib) and congestive heart failure (CHF). Using a patient-centered, evidence-based approach, the program empowers patients to better understand their condition, improve symptom management, reduce the risk of preventable hospitalizations, and enhance their overall cardiovascular health and quality of life.
Care is provided by a Heart Health Educator in collaboration with the patient’s primary care physician or nurse practitioner to ensure coordinated, high-quality cardiovascular care.
Program Services
As part of the program, the educator will:
- Conduct a comprehensive assessment, including medical history, current symptoms, medications, lifestyle factors, and cardiovascular risk factors.
- Assess the impact of AFib or CHF on daily activities, functional status, and overall quality of life.
- Provide individualized education about AFib and CHF, including disease progression, symptom recognition, self-monitoring, and strategies to prevent complications and exacerbations.
- Review prescribed medications to promote safe and effective use, improve adherence, and address questions regarding treatment.
- Provide education and support related to heart-healthy nutrition, regular physical activity, smoking cessation (when appropriate), weight management, stress reduction, and other lifestyle modifications that support cardiovascular health.
- Teach patients how to recognize early warning signs of worsening heart failure or atrial fibrillation and when to seek medical attention.
- Develop a personalized Heart Health Action Plan to help patients recognize changes in symptoms, respond appropriately, and support effective self-management.
- Coordinate referrals to community resources, cardiac rehabilitation programs, cardiology services, or other healthcare providers when appropriate to support comprehensive cardiovascular care.
