What is a Family Health Team?
A Family Health Team (FHT) is an approach to primary health care that brings together health care providers to co-ordinate the highest possible care for you – the patient. Designed to give family physicians support from other complementary professionals, the AFHT consists of Family Physicians, Nurse Practitioners, Dietitians, Social Workers, Registered Practical Nurses and Support Staff working collaboratively, utilizing their skills and experience so that you the patient receives the very best care when you need it and as close to home as possible.
Who can see your allied health professionals?
Only patients of the AFHT have access to our Allied Health Professionals once their physician makes a referral. Unfortunately, patients of outside physicians are not able to make appointments, even if their physician makes a referral.
How do I get test results?
To obtain test results, please book an appointment with your physician. Or, you may call and speak with one of our nurses. If the nursing team cannot address concerns over the phone they will suggest that you book an appointment with your physician.
How do I book a follow up appointment?
If your doctor wants you to book a follow up appointment, make sure you book your appointment with reception before you leave the office. Many times, the physicians are booking a few weeks in ahead and we need notice to make sure you are seen in the proper time frame.
How do I cancel an appointment?
Please call our office, and press #3 for cancellations and leave a message stating your name, health care provider and date and time of your appointment. It is important to cancel as soon as possible if you cannot attend as we have many patients waiting for appointments.
What is your referral policy?
If you would like a referral to see a specialist, this must be discussed with your physician. New referrals are not done by phone.
How do I get a prescription renewed?
Always bring all your medications to your appointments in their original containers to review with your physician. Make sure that you have enough medication to last until your next appointment. Prescriptions will be renewed at your appointment and can be sent directly to your pharmacy.
How do I become a patient of the Amherstburg Family Health Team?

What happens if I miss an appointment?
You may be billed for missed appointments.
Is the Amherstburg Family Health Team a walk-in clinic?
No.
Can I be seen without an appointment?
You must book an appointment to connect with a health care provider. For an appointment during the day for an urgent issue, please call our reception staff to determine our same day availability.
Do you have a lab?
We currently do not have a lab on site, however there are several labs in close proximity to our office:
Med Labs of Windsor (Amherstburg Site) located at 503 Sandwich St. South Unit # 5
Amherstburg Blood Lab (Inside Amherstburg Healthcare Centre) located at 433 Sandwich St. South Amherstburg
Are all services covered by OHIP?
The following services are not covered by OHIP and there may be a fee:
- Forms such as insurance papers, work or school notes
- Drivers physicals
- Faxing and photocopying when provided for a third party or patient’s personal use
- Transfer of medical records at the request of the patient
Any questions should be discussed with your physician.
Do I need to bring my OHIP card to every visit?
Yes. Your valid OHIP card allows us to ensure your current eligibility for OHIP.
What happens if my health card expires?
Please go to the nearest Service Ontario location to renew your card, you can also renew your health card online.
What do I do outside of office hours if I need care?
If you are experiencing a medical emergency, please call 9-1-1 or go immediately to the nearest Emergency Room.
If you have an urgent medical issue that cannot wait until tomorrow but is not a medical emergency, you can call our After-Hours Physician Line at 519-736-7770 Monday-Friday 4:30-8pm. On Saturdays, an in person walk-in clinic is available for AFHT patients only.

Who do I contact if I have a complaint?
