We are pleased to be a collaborating partner in the 1st Annual Amherstburg Community Flu Shot Drive-Thru Clinic.

Please print, complete and bring the following forms with you prior to attending the drive-thru clinic. The COVID-screener is to be completed the morning of the flu shot drive-thru clinic (November 12, 2020).

We will also have these forms available to complete on site if you are unable to do so beforehand.
